If you are a bride and you take your husbands name, you need to contact the Social Security Administration to notify them and request a new Social Security card.

Should you become divorced, it is necessary once again to contact the SSA with notification of any name change and a new SS card request.

This is important because when you file your taxes, names and social security numbers will not match and will only cause delays.

Form SS-5 is available at www.socialsecurity.gov or at your local Social Security Office. You can find your local office at their website or you may call 1-800-772-1213.

Your dependents should also have a social security number. If you have adopted children without a social security number you can fill out an adoption taxpayer identification number, or ATIN, Form W-7A with the IRS. The ATIN replaces the SSN on your tax return.
